Switching from wanting a noncompetitive specialty to a competitive one, relatively late in the game?

2023.06.02 17:05 alittlefallofrain Switching from wanting a noncompetitive specialty to a competitive one, relatively late in the game?

I’m an M3 three weeks into my OB/gyn rotation and absolutely loving it so far especially L&D. I was expecting to dislike it—I’m pretty type B, not competitive or ambitious, didn’t think I was good with my hands so had mentally ruled out surgery, also am male—but idk something has kind of clicked. Going into med school, i did exactly the thing everyone says NOT to do: I assumed I’d only want noncompetitive specialties, namely neuro or IM, so I spent preclinicals mostly chilling. I go to a big mid-tier US MD school; I remediated a block my first year and have had middling academic performance since then; no research (I don’t like research, I’m for sure not interested in a research heavy career) aside from being peripherally involved in a long-term neuro project that might take a while to get off the ground; no ECs. I was very certain that work life balance is more important to me than anything but idk about that anymore. In hindsight, that was dumb, and everyone should treat med school like they’re trying to go neurosurg or derm lol, but oh well I can’t change any of that now. Haven’t taken any shelfs yet or received any clinical grades but I will absolutely be grinding now.
I guess my question is, is there any point in further exploring ob/gyn as a possibility or have I already kind of screwed myself? Has anyone switched from zero interest in surgery to a surgical specialty later on in med school? I’m like trying to convince myself to not enjoy this rotation so much rn because I don’t want to get my hopes up lol, but if it’s a possibility then I would like to be able to start working toward that as soon as possible.

Left unanswered in this story about a Catholic health system taking over an independent OBGYN practice is how it will impact access to full spectrum reproductive care
Providence Swedish South Puget Sound, which operates Providence St. Peter Hospital, has acquired Olympia Obstetrics and Gynecology, the two organizations announced Thursday. The obstetrics practice will change its name to Providence Swedish Obstetrics and Gynecology Olympia. “Our mission and vision aligned, leading us to our decision to work in an employed specialty practice model,” said Dr. Laurie Sorenson, a founding partner of Olympia Obstetrics, in a statement. “Together, we will be able to ensure that our community has access to the highest level of specialty services for women for years to come,”
Sorenson also serves as the medical director of Women’s and Children’s Services at Providence St. Peter Hospital. Olympia Obstetrics and Gynecology PLLC formed following the closure of Memorial Clinic in 2001, according to a news release. “As health care changes in our country, it is becoming more difficult for independent physician practices to recruit and grow their services,” a news release reads. “Providence Swedish Obstetrics and Gynecology Olympia will work to recruit OBGYN physicians and will continue to look at avenues to grow the breadth of services."

2023.06.02 11:40 Sir_RADical Asking for a letter of recommendation at the end of a core rotation in Med 3?

I just started my third-year rotations, and my first rotation is in psychiatry which is my desired specialty. I've been doing well on my rotation so far and even had one of the patients speak well of me in front of the attending and residents during rounds! I was considering asking that attending for a letter of recommendation at the end of the rotation. Would it be a good idea to ask for a LOR so soon during my clinical years? Would the letter still be considered "valid" when applying for residency later on?

The article lists several top hospitals in Dubai, including Saudi German Hospital, Emirates Hospital, Latifa Women and Children Hospital, King's College Hospital Dubai, Rashid Hospital, Canadian Specialist Hospital, and American Hospital Dubai. It is important to consider factors such as location, medical specialties, and level of accreditation when choosing a hospital. Dubai also has a large medical complex called Dubai Healthcare City, which includes hospitals, clinics, teaching and research facilities, and partnerships with international institutions. Expats who wish to use public hospitals will need to apply for a health card from the Department of Health and Medical Services.

2023.06.02 02:15 Zurxor 3rd cycle school list?

3rd round for applying because I am an absolute buffoon of a human being. Last cycle I got 4 interviews, currently on 2 waitlists (definitely not getting in at this point). Anyone have some recs what schools to apply to for an extremely mid application? I have nothing besides the most generic ECs and I have no clue how to make myself unique besides the generic emergency surgery essay in my primary.
Yes, I have worked on my writing. Apparently advisors and the writing center were not the place to go, Reddit said I should just trust Reddit's criticisms for my essays only. I have to work on interviews too but it obviously isn't the only reason I am not getting in.

2023.06.01 21:33 Responsible-Bug-7464 Please help me decide! SLU vs CUSM

I have recently gotten off the waitlist at CUSM, and since I am a CA resident, It's made the decision quite difficult for me. Please help me! I'm interested in potentially pursuing IR, Ortho, ENT, and don't want to close any doors.
SLU: Pro - Better match-list for the specialties I'm interested in. - Close to WashU for research - Better Clinical rotations in home hospitals - Home programs for most specialities - 120K scholarship bringing costs to about 60K/yr + CoL - P/F grading
Con - Biggest con is far from family - Don’t know if I like St. louis and the location - Have no relatives/family nor friends nearby - I hear research is not spectacular at SLU
CUSM:
Pro - near family and my support system - I can use my phlebotomy/EMT licenses to volunteer at clinics - Have friends that go here - P/F grading - Gives extra time to prepare for Step exams and lectures are optional
Con - Terrible matchlist (Mostly IM, FM, and EM with barely any surgical matches and no Rads) not sure why - No home programs - No home hospital, rotates in random hospitals all around CA most of which aren’t very highly regarded - No research opportunities (on my own to fnd my own research) - About the same cost at 60k/yr + CoL

2023.06.01 21:05 bupr3me Gap Year Advice

Hi everyone! I recently graduated and am planning on applying next May for the 2024/2025 cycle. I have things planned out for this year, but I’m not sure if it’s enough since I’m trying to catch up. I’d appreciate it if you guys told me if what I’m doing is too little or even too much, and also some general advice/info on what you’ve done with your gap year(s).
Current stats:
Obviously I’m short on volunteering and clinical. I plan to address these the most during this next year before I apply:
In addition to this, I was planning on retaking the MCAT since my score is slightly below the median of the schools I want to apply to and my GPA is average. However, I’ve been told a 514 is a solid score so I’m hesitant about retaking.
There’s also more I’d like to do with my gap year to make myself stand out. I want to find something medicine-related that I’m truly passionate about, whether it be a specific research area or social cause. But I’d also still like to have a life outside of medicine and continue to pursue my hobbies.

2023.06.01 20:27 ihavecloroxwipes Looking for psychiatrists specializing in ADD/ADHD in the area

Does anyone have any recommendations? Or even general psychiatrists that could possibly have knowledge in this specialty? There are many ADD/ADHD psychiatrists here but very few specialize in this area and I'm getting a bit frustrated. I prefer to see a specialist in person and avoid any telehealth appointments. Checked with my insurance to see which ones are covered under my insurance. The only options are providers who are part of the Bay Area Clinical Associates group (BACA), but it looks like there have been no available appointments on their website. Sucks also that it seems like their focus are more towards child psychiatry and telehealth. Bit confusing. Thanks!
Sincerely,
A struggling individual in her mid 20s losing hope and struggling to make it through the day
Edit: looking for in-person consults in San Jose, CA

2023.06.01 20:24 Artichil Rads vs Peds

First, I know these may look like very different specialties but for me they both are “general” specialties that cover all systems of medicine which I enjoy and they both have opportunities for a good lifestyle. I have completely cut out anything surgical and can’t see myself limiting myself to a specific subspecialty as of yet. My goal is to ultimately work part time in the future and both of these seem to have good options for that as well. If I did peds I would likely do an outpatient private clinic.
I’m fresh into M3 so I’d like to know what to pay attention to or look out for to help make my decision. I’ve done part of my pediatrics rotation and it was an adjustment but I don’t have much to compare it to. So far I feel like I’d much rather go back to M1-M2 where I had my own schedule and didn’t have to deal with so many little day to day things that come with working with different people all the time and not having much autonomy or knowledge. I hope to do a radiology elective soon but don’t know how much insight it will give me with just two weeks and not having much knowledge. I finished preclinical in the second quintile of my class and do have some radiology and community service research as well as some leadership in clubs. In terms of my personality, I can get along with patients but it does take an extra effort for me to do that and doesn’t come naturally. The one thing I’ve found myself to really like is being able to be a little hands on with short/simple procedures (even if it’s just an ultrasound or a lumbar puncture or something) but not to the level of surgery.
Radiology Pros: - I truly find the technology and the fact that you can look into someone’s body super cool, especially ultrasound - I like the systematic approach as I’m a very organized person - Being able to diagnose things that clinicians couldn’t do without you - Some procedures (biopsy’s, etc) which I do think I’d enjoy from time to time - I don’t mind not getting recognition from patients so that can be a pro - Don’t have to deal with annoying patients, social work, don’t take your job home with you
Cons: - 6 years of residency which also means I probably won’t be able to start a family until my 30s - Mentally taxing and I’ve heard it’s just as bad or worse as an attending - Along with that, don’t really have down time when you are on the job, always rushed to read - Getting super competitive to match - Difficult board exam and I’m not sure if I’m smart enough for it - Threat of AI
Pediatrics Pros: - Only 3 years of residency - Easier to match, I’d love to just have to pass my rotations and do okay on step 2 and still be fine - You get to see cute kids and follow them for life, not a super big motivation for me but I see the appeal - I like the systematic way that u assess a patient based on their age - As an attending it seems like the job is fairly routine and easy and you’re not always doing something that uses all of your mind
Cons: - The things that come with clinic life such as the social work and lots of forms - Parents that are anti vax or make care difficult - Children that are difficult to exam - Can’t work from home but that’s okay if I can still go part time - Pays less but that’s not really a big factor for me
I guess I’d just like to know what are the big questions I should be asking myself to decide and what to pay attention to. Maybe a part of myself is tired of all this and just wants to pick something with a short residency and be done sooner but I guess it’s deciding if I actually like radiology that much or if I’d be just as happy in peds. It’d be great to hear I can just pass my rotations and do okay on step 2 and be fine but I guess I need the motivation to tell myself to work hard for these next few years and go for radiology and it’ll be worth it in the end.

2023.06.01 17:58 metal_ero Are we the baddies?

I never thought I would make a post here, since I have always been on the "other side". Let me explain: I am a psychologist and I work in education, and I have been collaborating with psychiatrists -as colleagues- for several years, recommending consultations to children and adolescents with whom I work.
All my life I have struggled with crippling anxiety, and in addition, I have always had the difficulties commonly associated with ADHD. I had never considered psychiatry, until a colleague made me realize that I could probably find some help in that space. I thought it could be a good idea.
Big mistake. And that's why I'm writing, I need to not only vent, but to know if anyone has had similar experiences, or if I'm overreacting.
The appointment was at 2:00pm. The psychiatrist arrived at 2:55, which had me pretty indignant because I had another appointment at 3:00pm for which I was invariably going to be late. When we went in, I just told her "I thought you weren't going to see me" and the doctor told me there was an error in the system, etc etc. She starts to ask me why I am there, and when I start to explain that I have suffered all my life from very high anxiety and attention deficit, she interrupts me to tell me that I cannot self-diagnose and that I was wrong because "attention deficit does not exist". Without giving her much importance, I continued trying to explain what is wrong with me.
One of my problems is that I can't finish anything I start (I'm oversimplifying, but just so you understand) and as soon as I said that, the doctor said "and why does that happen to you?", I didn't really know what to answer, and I told her "I really don't know, but that's why I came". She said to me in the most sarcastic and condescending tone: "Aren't you a psychologist? well, diagnose yourself! haven't you already been self-diagnosing? and how do you diagnose your patients? I didn't know where to start... what a cold and abusive way to receive someone suffering. I told him that I was not there as a psychologist but as a patient, ( I don't even work with patients, I'm just a school counselor) and that my ask was exactly the help I was looking for.
THAT WAS IT
From that moment on she was just typing, and then she told me that I have an anxiety disorder and a paranoid disorder. I was shocked, but in reality I had already decided internally that I was not going to take this professional seriously. She obviously gave me the prescription (you have to keep full the lab's pockets, right?) and without asking me anything else AT ALL, she sent me out with an antidepressant, two diagnoses, and feeling completely violated and mistreated in my need for help.
That was not the worst of it. When I reviewed her notes (in my country it is mandatory that when you leave a consultation with any specialty you are given a copy of your clinical history), the doctor wrote: "paranoid patient with self-reference ideas about punctuality, who is hostile and belligerent for the hour of attention, being irritable and combative during the interview".
So, and this is what I wanted to share with you, is it irrational to be outraged at being seen 50 minutes later, and then being bullied for being a psychologist and not knowing what is wrong with me?, and then, washing her hands of the whole thing saying that I am the problem because I am anxious and paranoid?
I shudder to think that those kinds of experiences can be someone's first contact with mental health care. At least I have the experience to know that I should just ignore this loser and seek another opinion, but I wanted to know if these types of experiences are normal and if anyone has experienced the same thing. I feel like the "Are we the bad guys" meme.
Sorry for the rant.

Looking at PNW but open to other areas. We’re a family of 3, late 40s-early 50s, my son is 11 and autistic, with global developmental delay and epilepsy. We currently live in Minnesota. We’re looking for a blue state, with plenty to offer my son. We need good schools with excellent special ed programs, specifically for autism. He will be entering middle school when we’re looking to relocate. Other requirements are, community programs such as Miracle League and programs for young adults aging out of public schools, specialty dentists for special needs children and adults, and excellent healthcare with specialty children’s hospitals and clinics. My husband is an engineer who owns his own thermoforming business. He’s in the process of either partnering or selling to another company so we may need to find work for him at a thermoforming company. Minnesota is great as far as politics, but our small town is too far from a lot of the resources my son needs. We’re also very done with the long, cold winters. We’d ideally like a more temperate climate. Four seasons are fine if the winters aren’t ridiculously cold and long. We’d like to be able to see mountains, or be near hike-able areas. We also enjoy kayaking and biking. We need mountains, trees, water. We’d like to be close to a major airport as we do travel and have adult children and extended family living in other states. We love going to concerts and comedy shows as well. Legal weed and LGBTQ+ friendly is also a want. With all this we also need affordable housing under $500k.

2023.06.01 15:38 DroperidolBro Deciding between job offers from two centres. Best advice regarding dilemma please?

Hi. Based in Australia rather than USA, so aspects of the system are obviously different, but looking for pearls to help me deal with a dilemma between 2 job offers. Just finished my training and been offered a consultant (attending) job at 2 different centres. Have spent time in each of them during training and like them for different reasons.
1 is a tertiary trauma centre, sees 80,000 patients per year, lots of opportunities to exciting non clinical portfolios (tox, ultrasound, education etc). Day to day role is essentially supervising trainees though (rather than direct patient load). Every subspecialty in the hospital, which sometimes means when it gets busy that the more interesting procedural stuff gets delegated to inpatient specialties.
  1. More regional centre that sees 50,000 patients per year. Less academic ties, still reasonably high acuity though major trauma bypasses to a bigger centre. Half the shifts are supervising trainees and the other half are direct patient load. No sub specialties in the hospital and no ICU backup, so sick patients require stabilising and maintaining until transport arrives.
I think the clinical load of the smaller centre is way more up my street, but the flip side is that non-clinical interests/portfolios are what will protect against burnout longer term. Any thoughts?!
